Our patients confirm that they understand they are responsible for payment of all services and products provided to them on the date they are rendered. Atlas Chiropractic will file insurance claims on patients’ behalf with major medical insurance companies and personal injury protection policies. If a check or debit is returned for insufficient funds, a service fee of $25 may be charged.
