Our patients confirm that they understand they are responsible for payment of all services and products provided to them on the date they are rendered. Atlas Chiropractic & Wellness will file insurance claims on patients’ behalf with major medical insurance companies and personal injury protection policies. If a check or debit is returned for insufficient funds, a service fee of $25 may be charged.
